ESPOC Confidentiality Agreement Template
This template establishes expectations for confidentiality within an MDP. A confidentiality agreement is not required between members of an MDP but reflects best practices. Pilot sites should modify this template to suit your own needs. It defines what constitutes confidential information, outlines rules for securing case materials, restricts the use of identifiable case details, details requirements for returning materials, and clarifies processes for releasing findings. It also instructs members on how to handle professional misconduct or safety threats, reinforces compliance with mandatory reporting requirements, and specifies that violations may result in removal. It positions the Project Coordinator as the central point for questions or concerns.
